Registration for audition: • Registration by mail or in person only. Phone   and Fax will not be accepted. • Placement audition is required for all    new students. • Each member including current members must    re-register each year. • Send the form with your payment to:

• Confirmation letter will be sent by e-mail. Be sure to provide a valid,
  current e-mail address.

Fees for DSP member:

• Annual membership fee: $25.00 (September 2006 ~ May 2007)
• Tuition: $20.00/rehearsal class, total of 32 rehearsals.
• Materials fee: $50.00 (Prep Level/Level I), $250.00 (Level II)

* Charge for Annual Performing fee is additional.

• Payment will be into two checks ($320.00 each):

  The 1st payment is due immediately after your registration.
  The 2nd payment is due on December 15th, 2005.

• Discounts: $10.00 off for the second or third sibling members.
   The $25.00 annual membership fee will be waived after three consecutive
   years of membership; or pre-pay whole year’s tuition.

   Be sure to write member name and class code on your check.

• A $30 fee will be charged for any returned check.
